Where relevant, you agree to provide accurate and up-to-date medical information to assist in your care.
If you are attending under a GP care plan or other referral-based service, it is your responsibility to ensure a valid referral is in place prior to your appointment. Failure to provide appropriate documentation may result in standard consultation fees being charged.
You may bring relevant reports to your appointment or send them in advance to admin@nextphasephysio.com.au.
All consultation fees are payable at the time of your appointment unless otherwise arranged.
Fees may vary depending on the type and duration of the appointment. Any applicable rebates (e.g. Medicare, private health) are the responsibility of the client to claim unless otherwise stated.
We understand that life happens and plans can change. To help us provide fair access to appointments for all clients, we ask that you adhere to the following cancellation policy:

We understand that unexpected situations can arise. Exemptions may be made at the clinic’s discretion in cases of exceptional circumstances.
If you need to cancel or reschedule your appointment, please contact us as soon as possible—we appreciate your understanding and cooperation.
